Circular Letter No. 96-1260

RE: Policy Information Report - 7/1/90 through 12/31/90

As a result of the settlement between Western National and the Association (see Circular Letter 96-1258 dated July 23, 1996), MWCIA is offering its members a list of all policies which became effective during the period July 1, 1990 to December 31, 1990. This listing will be provided on floppy disks in a delimited record format suitable for import into Microsoft Excel or an equivalent tool. The report contains the policy number, carrier number, association file number (RISKID), employer name, policy effective date, policy expiration date, applicable experience modification effective date, experience modification expiration date, modification factor, and the estimated premium reported on the policy.

You may mail or fax your written request for this report to Marc Coleman at the above address or fax number. Please provide the names and NCCI-assigned numbers for all insurance companies along with their group names and numbers. There will be a handling charge of $100 per hour with a two hour minimum. For billing purposes, please provide the name and address to be billed or your MWCIA charge account number. The floppy disk will be mailed within 10 working days from the date the written request was received.
