How do I obtain an experience modification when moving from a self-insured program to the voluntary market?
Experience of self-insurers may be included in an experience rating. The data must be submitted to MWCIA in an approved format (ERM-6). Refer to the ERM-6 form found under "Forms/Manuals Publications" tab, "Forms", at www.mwcia.org for instructions and along with a copy of this form.

Are E-mailed or faxed responses to MWCIA criticism letters considered electronic submissions?
No. If you are currently approved to submit policies, cancellations, reinstatements, and change transactions electronically, then submit your response through CDX. If you are not approved to submit data via CDX, then send your responses, hardcopy, through the US Mail.

What data is needed by MWCIA to experience rate an employer exiting a self-insured market and entering the voluntary market?
The same scope of data provided by the voluntary market is needed from the self-insured market. Refer to the MN Unit Statistical Plan Manual for details.

What are the 2009 and 2010 minimum and maximum payroll figures for sole proprietors, partners and officers electing coverage?
2009--$19,760 minimum and $88,400 maximum; 2010--$22,360 minimum and $91,260 maximum;

What are the 2009 and 2010 minimum payroll figures for the spouse, parent or child of a sole proprietor, partner, or officer electing coverage? What are the maximum payroll figures for these individuals?
The minimum payrolls are:
2009-$255.00 per week or $13,260 annually. 2010-$263.00 per week or $13,676 annually.
Payroll of these family members is not subject to a maximum amount.

What is the State Per Claim Accident Limitation amount for 2009 and 2010 experience modificatons?
2009--$171,500; 2010--$172,000;

We did not receive a copy of a Unit Statistical Report or any Rejection Edit Errors for April, 2009. Does this mean that all of our records were accepted as okay?
MWCIA discontinued sending "hard copy" reports and rejection letters relating to USR data effective January 1, 2009. These reports are now available to members via MWCIA’s website as part of our Manage USR program. Please refer to Circular Letter No. 08-1533 for further details regarding this change.

Can I request a Certificate of Insurance from MWCIA?
You can attach a request for a Certificate of Insurance to an online or hard copy application. MWCIA does not issue the certificate. The request is forwarded to the servicing contractor along with the assignment. If the application was submitted without the request for a certificate, you should contact the servicing contractor.
